Your Private Physician Lab Orders

The Cal Poly Health Center laboratory will assist students who come to the lab with complete laboratory orders from their personal physicians.

Students must bring formal and complete orders for lab work from their physicians, along with up-to-date billing and private health insurance information if you desire to have your insurance billed. Bringing their insurance card along is very important and copies of both sides of the insurance card must accompany the order when arriving at the laboratory.

These specimens are sent to Central Coast Pathology Laboratory and are NOT performed in the Cal Poly laboratory.  The results will be faxed directly to your doctor.

The Cal Poly laboratory does not receive these results and are not part of the

student’s health record here at the Cal Poly Health Center.

In order to process your lab order and comply with regulatory requirements, the following information MUST be on the lab order you bring to the laboratory:

The doctor’s lab order with appropriate ICD-9 codes or narrative, clearly defined tests requested to be performed, patient’s full name, date of birth, permanent address, and phone number, Doctor’s signature, Doctor’s name, address, office phone number and fax number.

If any of the above information is missing from your order, please contact your doctor and have the information faxed to Cal Poly Health Center Laboratory at (805) 756-5416 and Central Coast Pathology Laboratory at (805) 785-0432.

Central Coast Pathology Laboratory will bill your insurance if accurate insurance accompanies the lab order. A copy of both the front and back information on the insurance card must be presented at the time of the visit.

CCPL does not accept Kaiser Permanente or CCPN insurance. CCPL does provide a 50% discount for paying at time of service. You may obtain pricing by calling the Cal Poly Health Center at (805) 756-5271.
The Cal Poly Heath Center will collect lab specimens and send them with your payment to Central Coast Pathology laboratory. CCPL will send the results directly to your physician.

If you would like a copy of your results you may fill out a consent form and mailing envelope at the Health Center and a copy will be mailed to you.

You can contact Central Coast Pathology Laboratory directly for current pricing, additional information, or billing questions at
