IMMUNIZATION REQUIREMENTS

New Fall  2009 students:

Take action now to complete the necessary immunizations and send your records before classes begin to:

A copy of any one of the following will satisfy your immunization requirements

Campus Residents:  Students living in on-campus housing are required to submit a Meningococcal Response Form to Health Services.  Note:  The Meningococcal Response Form is in addition to the Measles, Mumps, Rubella and Hepatitis B Immunization requirement.

If you fail to complete the immunization requirements, the following could occur:

  1. A registration hold that prevents a student from registering for the next quarter.
  2. In the event of an outbreak of a covered illness, those who have not received the necessary immunizations will be barred from campus until the outbreak is over.  Two to three weeks of missed classes, final exams, commencements, and other events of significant importance could result.

We highly recommend you complete all required immunizations form your family doctor or local public health department (where they are often available at a lower cost than the Student Health Center can provide).

Hepatitis B Immunization Requirement

All new Cal Poly students who are 18 years of age or younger on the first day of classes of the first quarter of enrollment at Cal Poly must present proof of Hepatitis B immunization.  The Hepatitis B immunization is a series of three shots, usually given over a period of six months.  You will NOT be allowed to register for your second quarter until you have presented proof to Health Services that you have received at least one shot.  You must complete the series prior to your second year of enrollment. Students enrolled in a California public school for the seventh grade or higher on or after July I, 1999 will have satisfied this requirement.

Measles/Rubella Immunization Requirement

All students born after January 1, 1957 must show proof of full immunization against measles and rubella.  Certain groups regardless of age must also show proof of full immunization.  These include those who will live in the campus residence halls and all nutritional science or education majors.  Proof of immunity to measles and rubella must be submitted within one year of the time of first enrollment or you will NOT be allowed to register. Students enrolled in a California public school for the seventh grade or higher on or after July I, 1999 will have satisfied this requirement.
